How to reduce patient no-shows at your clinic

An evidence-informed reminder workflow for private clinics: confirm appointments, send timely reminders, make cancellation easy, and measure attendance.


The most reliable way to reduce patient no-shows is to confirm the appointment when it is booked, send a reminder before the visit, and give the patient a simple way to cancel or reschedule. The clinic also needs one accurate appointment list so reception can see replies and refill cancelled slots.

What does the evidence say about appointment reminders?

Evidence supports text-based appointment reminders across healthcare settings. A systematic review and meta-analysis published in BMJ Open included 26 articles. In the pooled analysis, patients who received electronic text notifications were more likely to attend and less likely to miss appointments than patients who received no notification.

The same review found that multiple notifications performed better than a single notification. A separate Cochrane review of mobile messaging reminders also found that text reminders improved appointment attendance compared with no reminder.

These studies evaluated text messaging broadly, not Clinic Grid or WhatsApp specifically. They support the reminder principle; each clinic should still measure its own attendance, cancellations, and patient preferences.

A practical reminder workflow for a private clinic

1. Confirm the appointment immediately

Send the date, time, doctor, and clinic location as soon as reception books the appointment. A written confirmation is easier for the patient to check than details given only during a phone call.

2. Send a reminder before the visit

A reminder should repeat the essential details and explain how the patient can confirm, cancel, or ask for a different time. Keep it short enough to understand at a glance.

3. Use a second reminder only when it helps

For appointments booked well in advance, a second reminder on the day of the visit can bring the appointment back into the patient’s plan. Avoid sending unnecessary messages that patients may start ignoring.

4. Make cancellation easy

A patient who cannot attend should be able to reply without making another phone call. Reception can then update the schedule and offer the opening to another patient.

5. Keep appointments and replies in one workflow

Reminders work better operationally when reception can see the appointment, confirmation status, cancellation, and queue in the same system. Otherwise, staff still have to reconcile a diary, phone calls, and separate WhatsApp conversations.

Use only the information the patient needs:

  • clinic name
  • doctor’s name
  • appointment date and time
  • location or directions link
  • a clear confirmation or cancellation action
  • the clinic’s contact number

Avoid placing unnecessary health details in a general appointment reminder. The message should help the patient attend without exposing more information than required.
